{"id":28985,"date":"2021-11-18T12:31:11","date_gmt":"2021-11-18T11:31:11","guid":{"rendered":"https:\/\/www.coflaspalmas.es\/?p=28985"},"modified":"2025-11-12T10:13:24","modified_gmt":"2025-11-12T09:13:24","slug":"el-consejo-general-de-farmaceuticos-reconoce-la-participacion-del-coflp-en-el-proyecto-concilia-2","status":"publish","type":"post","link":"https:\/\/www.coflaspalmas.es\/de\/el-consejo-general-de-farmaceuticos-reconoce-la-participacion-del-coflp-en-el-proyecto-concilia-2\/","title":{"rendered":"Der Allgemeine Rat der Apotheker erkennt die Teilnahme des COFLP am Projekt \u201cConcilia 2\u201d an\u201d"},"content":{"rendered":"<p class=\"Standard\" style=\"text-align: justify; text-justify: inter-ideograph;\"><b><span style=\"font-size: 15.0pt; font-family: 'Helvetica',sans-serif;\">Gemeinschafts- und Krankenhausfachleute arbeiteten aktiv zusammen, um Medikationsfehler nach der Entlassung aus dem Krankenhaus zu erkennen.<\/span><\/b><\/p>\n<p>Der Allgemeine Rat der offiziellen Apothekerkammern hat die Teilnahme der Apothekerkammer von Las Palmas (COFLP) an der Forschungsstudie \u201cConcilia Medicamentos 2\u201d \u00fcber den Service der medikament\u00f6sen Abgleichung bei Krankenhausentlassung anerkannt, der von Apothekern verschiedener Versorgungsebenen koordiniert wurde. Diese Studie, die vom Allgemeinen Rat der Apothekerkammern geleitet wurde, hatte zus\u00e4tzlich die Beteiligung der Universit\u00e4ten Salamanca und San Jorge (Zaragoza), die Zusammenarbeit der Laboratorios Cinfa und die Teilnahme von sieben weiteren Kammern aus verschiedenen Provinzen.<\/p>\n<p>An der Studie nahmen insgesamt 145 Apotheken und 10 Krankenh\u00e4user teil, mit insgesamt 204 Gemeinde-, Krankenhaus- und\/oder Prim\u00e4rversorgungsapothekern und 622 Patienten. Ihre Ergebnisse zeigen deutlich, dass die Intervention des Apothekers durch den Medikationsabgleich bei Entlassung aus dem Krankenhaus wirksam ist, mit mehr als 860 Interventionen, davon 80%, die vom Arzt akzeptiert wurden und die die Versorgungskontinuit\u00e4t f\u00fcr \u00fcber 65-J\u00e4hrige, Poly-Pathologiker und Polimediziner, die etwa 10 Tage station\u00e4r behandelt worden waren, um 96,2% erleichterten.<\/p>\n<p>In der Fallstudie f\u00fcr Las Palmas wurden von den 40 eingeschlossenen Patienten 95% in \u00f6ffentlichen Apotheken versorgt, w\u00e4hrend die verbleibenden 5% in Krankenhausapotheken versorgt wurden, mit anschlie\u00dfender Betreuung in \u00f6ffentlichen Apotheken. Dar\u00fcber hinaus zeigten 97,5% mindestens eine Diskrepanz bei ihrer Entlassungsbehandlung, verglichen mit 96,5% in der gesamten Studie.<\/p>\n<p>Dieser Dienst hat sich als wirtschaftlich tragf\u00e4hig erwiesen. In mehr als 90% der F\u00e4lle wurde der Patient nach der Leistung nicht wieder ins Krankenhaus eingeliefert oder musste auch keine Notaufnahmen aufsuchen.<\/p>\n<p>Was das globale Profil der Studienteilnehmer betrifft, so waren 70,3% \u00fcber 65 Jahre alt, 88,3% polypharmaziert (mit mehr als 5 Medikamenten) und 56,6% m\u00e4nnlich. Die Entlassungseinheiten waren haupts\u00e4chlich Innere Medizin, Kardiologie oder Traumatologie, und alle wiesen mindestens ein Gesundheitsproblem und ein Medikament mehr auf als bei ihrer Aufnahme.<\/p>\n<p>Insgesamt identifizierten die Apotheker 2515 Abweichungen, was einem Durchschnitt von 4,2 Abweichungen pro Patient entspricht, und 739 arzneimittelbezogene Probleme (AMP), mit einem Durchschnitt von 2 AMP pro Patient. In Las Palmas identifizierten die Apotheker 145 Abweichungen und 105 AMP. Die h\u00e4ufigsten AMP waren mangelnde Therapietreue, Wechselwirkungen und mangelndes Wissen \u00fcber die Anwendung des Medikaments. Die Identifizierung von AMP durch Apotheker in der Gemeinde ist hoch, was die Notwendigkeit einer intensiveren Patientenbetreuung w\u00e4hrend der \u00dcberg\u00e4nge in der Versorgung zeigt.<\/p>\n<p>Diese Studie zeigt die Bedeutung der Koordination und Zusammenarbeit zwischen Gemeinde- und Krankenhausapothekern. Die patientenorientierte Arbeit von Krankenhausapothekern ist wesentlich f\u00fcr die Identifizierung und L\u00f6sung von Diskrepanzen. Ebenso erm\u00f6glicht die Kontinuit\u00e4t dieser Patienten in der Gemeindeapotheke die Kl\u00e4rung von ungel\u00f6sten Diskrepanzen und die Erh\u00f6hung der Erkennung von Fehlern bei der Abgleichung.
